District Nurses -DN

To lead and manage a team in the provision of a community nursing care for a defined Community locality.

Establish high quality programmes of evidenced based care for individual patients referred to the team.

Ensure the implementation and evaluation of care in conjunction with other agencies incorporating statutory and voluntary services.

Manage a team with varying skill mix. 


Clinical Responsibilities

Ongoing responsibility of a defined caseload

Responsibility for the organisation and management of the Community care team

Accept referrals from health and social care professionals and voluntary organisations and self-referrals within a defined population.

Responsibility for the assessment, implementation evaluation and reassessment of care to all patients referred to the team.

Plans programmes of care for patients with diverse and complex health needs

Plan workload to meet changing need of the patients.

Primarily visits patients in their own homes or another place of residence eg: residential care home.

Responsible and accountable for delivery and organisation of clinics to meet the changing needs of the service, e.g.: dressing and phlebotomy clinics.

Discharges and/or refers on individual patients from the caseload when they no longer require the input of the Community care team.

Works flexibly to meet patient and service needs.

Support for other Community care teams, in the absence of their team leader

Responsible for team duty rota, and their adjustment as necessary

Identify unmet need in the service/health care provision to patients within the defined area of practice and supports the Clinical locality manager in meeting these needs.

Provides health promotion information and advice to patients, carers and families.

Maintain excellent knowledge of developments in care and service provision within the district nursing service.

Travels to visit patients and others within the community to fulfil caseload responsibility.

Registered with NMC

Post Registration District Nurse Qualification

V100, Community Nurse Prescribing qualification

ENB 998, Mentorship qualification

Degree in relevant subject, working towards or willing to undertake.

Evidence of evidence-based practice.

Ability to travel within the community.

Ability to work autonomously.

Flexible in approach

Motivated and enthusiastic

Clinical role model

Ability to work on own initiative.

Must have a Full UK driving licence and access to own transport.
